The GAMHPA resource directory is distributed to over 1,200 professionals in the community, including mental health professionals, physicians, schools, attorneys, and churches.
Lake Avenue Community Foundation
Health Services Team
Lake Avenue Community Foundation is a 501(c)3 non-profit corporation that is committed to transforming the San Gabriel Valley by developing and supporting initiatives in the areas of housing, education, literacy, hunger relief, healthcare, and economic development. The Foundation was born out of the vision of the leadership of Lake Avenue Church (LAC).
The Lake Avenue Community Foundation was created March 1, 2001 to acquire and provide resources in support of a wide variety of programs assisting the poor and disadvantaged throughout the San Gabriel Valley.
Health Services Team
The Health Services team equips people to take care of their own health — mind, body and spirit — and to become ministers of health to others. Sanctity of Human Life stands in the gap for those who cannot stand for themselves: the unborn, the disabled, those in a vulnerable social position, and those who are victims of disease, abuse or other limiting conditions.
Help in Finding Resources
The first Sunday of each month after the 9:00 am and 11:00 am services in the Worship Center lobby of LAC, Lorraine Cummings, LCSW,
C-ASWCM of the Health Ministry Team and a professional Geriatric Care Manager, offers free consultation to the LAC community to connect people in need to a holistic array of resources to support their health and well-being.
